To open a mailbox you can either visit our office between the hours of 8:00 am. and 4:00 pm. Monday through Friday or 8:00 am. to 12:00 pm. Saturday and the clerk on duty will be happy to assist you. However, sometimes it is necessary for the clerk to step out of the office, so it is advised to call for an appointment first. Second, if a visit to the office is not possible, then just follow the four simple steps as listed below and you will have a mailbox:

  • First – Download and fill out the following forms:
    • The Mailbox Waikiki Rental Agreement
    • USPS 1583 Please read entire section before filling out form. For clarity with this form and how to fill it out properly, please click HERE.
      • What is the USPS 1583?
        • Since the U.S. Postal Service regulates how mail can be received and handled in the United States. it also dictates required procedures for commercial mail receiving agents (CMRAs) like The Mailbox Waikiki to follow. The USPS Form 1583 is a document that proves you have authorized a CMRA to receive mail on your behalf. Therefore, the 1583 is required before a mailbox can be assigned for personal use.
        • This form must be filled out and signed by each person who expects to receive mail in the box. Furthermore, copies of two forms of ID are required and must be attached to the 1583. *One ID must contain a color photo.
      • Important Notes:

        • At least one ID must have a photo of the applicant
        • At least one ID must verify the home or business address
        • ID’s that lack both a photo and address are not acceptable
        • If a business is associated with your account, then a separate verification document must also be provided.
      • Following is a list of approved IDs:
        • Armed Forces ID
        • Valid driver’s license
        • Valid state ID
        • Government issued ID
        • University ID
        • Passport
        • Sam’s Club ID
        • Costco ID
        • Corporate ID
        • Airline or airport-issued ID (if issued under a TSA-approved security plan)
        • Federally recognized, tribal-issued photo ID
        • HSPD-12 PIV card
        • Alien Registration card
        • Current lease, mortgage, or deed
        • Voter registration card
        • Utility bill
        • Home or car insurance card
        • Vehicle registration card
        • Medical insurance card
        • Medical prescription card
      • Other Acceptable Documents Include:
        • Business license or articles of incorporation
        • Utility bill
        • Lease or mortgage
        • Any document or letter from a recognized governmental or financial institution to you and your business
  • Second – Form 1583 must be notarized.
    • Individuals authorized to perform this duty are:
      • Notary Publics
      • Justices of the Peace
      • Bank Officers
        • *Some banks offer free notary service to their customers, so you may want to check here first.
      • Lawyers
    • International/Non-U.S. Residents
      • Since banks and embassies normally have notary agents on duty, those who live outside the United States will find these institutions very helpful for this particular need.
  • Third – Email all following documents as attachments to:
    • registration@mailboxwaikikii.com
      • USPS 1583
      • Mailbox Rental Agreement
      • Copies of 2 valid IDs, which will be checked to assure compliance with requirements of Post Office.
  • Fourth – When agent is satisfied that all forms required are filled out correctly and notarized, then payment is made and mailing address is established.
